What is an Anal Fissure? An anal fissure is a painful tear in the sensitive skin tissue at the anal opening, giving patients a sensation of “glass shards” or a “knife cut” during and after defecation. This severe pain causes the anal muscles (sphincter) to contract excessively, entering a “spasm.”
The spasm disrupts blood flow to the area, preventing the tear from healing and turning the situation into a “pain-spasm-wound” vicious cycle. In our patients, this situation leads to a fear of going to the toilet and postponement behavior due to severe pain, consequently leading to further constipation and pain.
Laser Fissure Treatment in İzmir: Spasm Resolution Without Cutting Muscle In classical surgical methods (Sphincterotomy), the internal anal muscle is cut with a scalpel to resolve the spasm and relieve the patient. However, cutting the muscle brings the risk of gas and fecal incontinence in the future.
In the modern laser treatment we apply in our İzmir clinic, the muscle is not cut. Using the controlled thermal (heat) effect of the laser, the contracted muscle fibers are relaxed. Thus, the spasm is resolved without the risk of gas leakage (the patient’s worst nightmare), blood supply to the area increases, and the fissure heals rapidly.
How is the Treatment Performed? (Step by Step)
Preparation
Performed with local anesthesia or light sedation; it takes about 10 minutes.
Cleaning
The non-healing, hardened (fibrotic) tissue is vaporized with the laser.
Relaxation
The contracted internal anal muscle is relaxed using laser heat (spasm is resolved).
Result
Once the spasm is resolved, pain decreases immediately, and the fissure heals rapidly. There is no risk of gas leakage/incontinence.
